Your Clinical Records: No Charge

Your session notes are yours at no cost. You can request copies anytime.

I recommend reviewing your file with me in a session. Clinical notes use professional language and are easier to understand with context. This helps you get the most out of your records and ask questions. (Standard session rate applies.)

Prefer just to get copies? No problem - submit a request below, and I'll send them to you. Just know that clinical terminology might be confusing without discussion.

Cost: Free (printing/postage charges apply for mailed copies: $0.50/page + postage)

Administrative Services: Session Rate Applies

For services beyond providing your records, the following are billed at the standard session rate (30-minute minimum, billed in 15-minute increments):

  • Writing letters or reports within my scope of practice

  • Completing other forms

  • Extended emails or phone consultations beyond scheduling

  • Reviewing records together in session

How to Submit a Request:

Please include:

  • Your full name and date of birth

  • What you need (file copies, letter, report, form, etc.)

  • Specific details (dates, purpose, who it's going to, etc.)

  • How you'd like to receive it (secure portal, PDF email, or mail)

  • I will require you to sign a copy for my files of a consent to release information

What to Expect:

Timeline: Please allow up to 30 business days for processing. I'll let you know if I need more time.

Payment: Any applicable service fees are due before documents are released.

Delivery options:

  • Secure portal (fastest)

  • PDF via secure email (charges may apply)

  • Mailed paper copies (printing and postage charges apply)

Requesting Corrections:

If you believe there's a factual error in your file, you can request a correction. Clinical observations and professional opinions won't be changed, but if we disagree, you're welcome to provide a written statement that I'll add to your file.
